Transfer Forms:

  1. MO (Member Only Transfer)

Complete the CMA Option A Transfer and submit to the Home Office Coding Department.

Please email the completed form to your VP for review and forwarding to the Home Office.  If you have already obtained the position of VP or higher you are authorized to contact the home office directly by using the contact page on your back office.

The form must contain all required signatures:

  • Member
  • All Active Releasing Up-line (if necessary)
  • Releasing VP
  • Accepting VP
  • Releasing EVP (Level-30) or above
  • Accepting E VP or above

If any required signatures are missing, the transfer request will be rejected.

The Member’s entire down-line rolls up to the Member’s first active up-line and remains within the original hierarchy.  The down-line does not transfer with Member.

All debt of the Member will be moved to the new hierarchy.

  1. MD Transfer (Member and Down-Line Transfer)

The form must contain all required signatures:

  • Member
  • All Active Releasing Up-line (if necessary)
  • Releasing VP
  • Accepting VP
  • Releasing EVP (Level-30) or above
  • Accepting EVP (Level-30) or above

If any required signatures are missing, the transfer request will be rejected.  The Member’s entire down-line will transfer along with the Member to the new hierarchy.  All debt of the Member will be moved to the new hierarchy.

** NOTE ** In an effort to maintain the integrity within the hierarchy, when a VP (Level-20) or higher submits the Authorization for Contract Change form requesting a demotion, the demoted member will remain at the requested lower level for 12-months from the date of processing; EVP (Level-30) or above exceptions will not be authorized. In order to regain the previous VP (Level-20) status or higher, he/she must re-qualify. General transfer guidelines will apply. The preceding rule applies for all levels.
